How to make eye pics like this on an IPhone 4 s?

Ok so I have a iPhone 4 s I guess and I really want to make pics of my eyes in collages and make them look like a tumblr picture and I really need some good advice that's worth it like some free eye pic apps that really work so please answer asap!
